For this assessment, students are required to analyse and critically review a case of a deteriorating patient, which is provided below. As with every academic piece of work, the analysis and review should be supported by contemporary literature and referenced using APA 7 referencing style.

The Case
On November 4th, 2018, 79-year-old Adam Smith was admitted to hospital for a left total knee replacement surgery on 4th November 2018.
Past medical history:
Ischaemic heart disease
Hypertension
Type 2 diabetes
Osteoarthritis
Peptic ulcer
Medication:
Mr Adam’s current medications are:
Metformin 1000mg BD
Glipizide 5gm OD
Atorvastatin 20gm OD
Clopidogrel 75mg OD
Paracetamol 665 mg 2 tablets TDS
Pantoprazole 40mg

Current clinical status:
On 5th Nov at 0915 Adam vomited a large of amount of gastric contents that were coffee ground in colour. Following the vomiting episode, a registered nurse (RN) assessed him and recorded the following vital sign:
BP: 153/90
HR: 116 bpm
RR: 34 bpm
Oxygen sat: 84% on 4Littre oxygen per minute
Temperature: 37.2
LOC: Alert
Based on the vital sign observation parameters and hospital policy, the registered nurse (RN) looking after Mr. Adam decided to activate the Rapid Response Team (RRT). The RRT attended promptly and increased Adam’s oxygen therapy level to 8 litres per minute. The next set of vital sign observation was taken at 0925, noted as below:
BP: 158/94
HR: 112 bpm
RR: 26 bpm
Oxygen sat: 89% on 8 litres of oxygen per minute
Temperature: 37.2
LOC: GCS 13/15
Consultants from Gastroenterology, internal medicine, and intensive care unit (ICU) were contacted to review the patient and investigate possible gastrointestinal bleeding and other complications. Based on their assessment, the patient was transferred to the ICU for further support. Despite extensive and coordinated interventions, and escalation of care, Mr Smith passed away 3 days later.
